I think one of the factors as to why Canadian and some American systems do so well is that they HAVE to be efficient and due to economic cycles, they periodically have to do hard pruning of the system because funding does fall short.

On the other hand we have more funding (yes really we do) but nobody dare redesign the network for connections or
efficiency because there isn't that great a pressure to do so.
Negative people... have a problem for every solution.
