A bit of a post-mortem here:
I found the transport away from this event quite successful. Understandably chaotic as everyone left after the last fireworks went off.
As I exited to South Brisbane station, the signage was easy to understand and we were all corralled in to the appropriate area based on destination. I caught the second train and there was plenty of empty seating despite the huge crowds.

The only issues I had were with the frequency of ferries as I arrived to South Bank earlier in the day from North Quay. The new bridge from Queens Wharf was already closed as I arrived, so I waited for a ferry to come. There were too many passengers to fit in the first ferry and I believe I ended up waiting over half an hour.
