I would imagine the volume of feedback through the form would be significant.  Most organisations would wonder why it stopped?

The feedback process must be improved so that it generates an an email confirming receipt and a reference number as suggested by Dwb.

Good way to try to improve their statistics and to reduce the numbers of complaints they have to handle, tip the lot into the waste basket and shred them then use the we had an equipment failure excuse to those customers who bother to chase up their complaint when they get no response.
One can not be other than suspicious as even most people using home computers have a back up system and this effort is based upon Translinks record to date just a little too convenient for them. ;) :o ??? >:D
