Please feel free to add yours ...

1.  Failure to put in place a rocket rail bus Springfield to Richlands.

2.  Failure to route the rail buses substituting post floods Ipswich to Darra, from Wacol to Richlands rail rather than allow up to 30 minutes delay from Wacol to Darra.

3.  Failure to alert the public of the new frequency gains Darra to CBD post Richlands.  Confusing timetabling such that not obvious to many.

4.  Failure to gain public confidence with the looming rail timetable changes.  Secrecy is wearing thin ...

5.  General road and bus congestion meltdown.  Won't be fixed by more roads.

6.  Failure to put in options to allow pax to transfer out of Congestion (Coronation) Drive delays particularly at peak.

7.  Failure to direct that bus priority be re-established.

8.  Delaying Cross River Rail project.

9.  Passengers unable to board a bus at Roma St INB to Herston for up to 30 minutes or longer.
Half baked projects, have long term consequences ...

Failure to keep up with Perth on everything from TODs, Rail Frequency, Land use integration, Connectivity and bus-rail interchange, orbital bus frequency, rail tunnels in the CBD (Perth has two PLUS underground stations), moratorium on freeway building (perth has this, though haven't  been able to dig up the details), and costs per kilometre (building anything in Perth is so much cheaper over there, you really have to wonder why things cost so much over here).  :is-

Stillwater

Failure to have a costed transport plan

In addition to a 'large projects' infrastructure plan, failure to have a program of relatively low-cost, smaller projects that address efficiency and the productive use of rolling stock

Failure to do what the government says it will do (i.e. Beerburrum Landsborough duplication)

:is- Yes. 6 billion per year on capital expenditure, each year, every year for the next 20 years straight just is not going to happen. They would be lucky to get 10% of that funding, or 600 million per year in capital funding. I don't think even new tunnel roads have had that money poured into it.

Brisbane seems to have a serious concrete fetish, where the most extremely costly solutions are touted. How about some bus pre-emption at signals? Relatively cheaper than almost everything in Connecting SEQ 2031.

10. Failure to have reasonable city stop locations, and sheer bloody mindedness that they would start the P88 route rather than fix the problem.

11. Failure to put out of service bus routes, counter peak into service, particularly on the INB.

12. Failure to have a reasonable counter peak service at Milton, and South Brisbane/South Bank/Park Rd.  Same as above point really, but for trains.

13. Failure to have outbound 4xx expresses in the AM peak serve the Cultural Centre

14. Failure to pursue legal action against the BCC for reducing the bus then transit lane on Coronation Drive. (According to quotes on this site, there was an act of Parliament in force at the time that the govt must approve such moves).  Note: this could have been done or advice received that it would be unsuccessful

15. Failure to get a reasonable service on the Airtrain.

16. Failure to chose a reasonable end-of-duplication point on the Sunshine Coast.
